Peer Recovery Specialist/Wellness Coach

REPORTS TO : Partial Care Supervisor
Hours: Monday - Friday, 8:00am-4:30pm, 30-minute unpaid lunch, for 40 hours per week
Position Summary :
This position will provide primary care educational and support services to SMI individuals who are enrolled in the Behavioral Health Home (BHH) program.
Position Qualifications :
High school diploma or equivalent and experience in providing recovery support services
Certified by at least one of the following credentialing entities preferred:
Addiction Professional Certification Board as a Certified Recovery Support Practitioner (CRSP) or a Certified Peer Recovery Specialist (CPRS);
Mental Health Association in New Jersey as a Certified Recovery Support Practitioner (CRSP)
NAADAC, the Association for Addiction Professionals as a National Certified Peer Recovery Support Specialist (NCPRSS);
Certified Wellness Coach
Community Mental Health Associate certificate
Experience in provision of recovery-related supports to behavioral health populations preferred;
Personal lived experience;
Skills : Effective written and oral communication skills. Knowledge of computer systems and electronic health record. Organizational abilities. Strong customer service skills. Ability to work in a team environment. Experience with behavioral health populations. Personal lived experience and ability to instill hope and wellness. Ability to transport clients into the community to meet their social determinants of health needs.
Working Conditions :
Contact with clients under a wide variety of circumstances. Handles emergencies or crisis situations.
Full-time work hours are 8am-4:30pm Monday-Friday with a 30-minute unpaid lunch for 40 hours per week.
Ability to transport clients within the Behavioral Health Home program to physical and behavioral health appointments as well as address social services needs within the community.
Physical Demands :
Ability to lift 10 to 25 lbs.; bending to do filing; sitting for long periods of time.
Age Specific Knowledge and Skills :
Must demonstrate the knowledge of key principles of development of the life span; knowledge of recovery-based concepts and interventions; knowledge of psychiatric and medication assisted treatments.
Duties and Responsibilities :
Care management of a caseload of individuals with Severe and persistent mental illness (SPMI) enrolled in the program;
Completes timely documentation in accordance with AAMH documentation policy and procedures;
Complete wellness and mental health screenings;
Assist in care coordination with nurse and prescribers to ensure access to primary health care and compliance with behavioral health services;
Assist client in scheduling physicals, blood work as well as coordinating care to secure medical records, including discharges summaries
Provide transportation directly to clients for physical and behavioral health appointments and accompany them, as appropriate
Conduct visits within the community, including in client homes, as appropriate
Facilitate wellness and recovery groups, providing education on nutrition, community resources and other health related topics.
Promotes consumer-driven services and recovery-orientated environment; assists individual and their family in making decisions, taking action and treatment planning. Provides education and information to the individual and their family;
Support individuals in their self-chosen goals;
Focus on engagement and using their lived experience to guide individuals through crisis;
Participate in treatment team meetings and individual staff consultations for assigned consumers;
Provides rotating telephonic after-hours coverage for program clients;
All appropriate projects and additional assignments as requested by Supervisor.
